Important Medical and Athletics Forms
All student-athletes must have up-to-date medical forms on file before they are allowed to participate in athletic programs at Immaculata High School. Please contact Athletic Trainer Greg Watson if there are any questions at [email protected]. Click on the link below for all Medical Forms:
NJ Department of Education Health Forms - On the NJ Department of Education website, you will find the "Health History" Form and the "Annual Athletic Pre-Participation Physical Examination Form." These are for families and their physicians and do NOT need to be returned to Immaculata; please retain for your own records.
2024-2025 Complete packet of Medical/Athletic Forms - These forms must be filled out and returned to Immaculata for clearance.
2024-2025 Health History Update Questionnaire - This form must be filled out and returned to Immaculata for clearance.
Nursing Services
Nursing services are provided by The Educational Services Commission of NJ. The Health Office is open Monday–Friday 8 AM–2 PM. The Health Office is closed over the summer.

Physical Education Option II Application:
All Option II applications are due to Mrs. Boczany by August 30, 2024 (for full-year or first semester) / February 7, 2025 (for second semester). No applications will be accepted after that date.
Recognizing that our student athletes are involved in rigorous academic and athletic programs, the Immaculata High School administration and physical education department have designed an option for student athletes to be exempt from PE classes while in season. Students who qualify may complete the Option II Physical Education Application and return it to the Academic Office.
